So this happened when the power went out. Upon rebooting, I restored my previous Firefox session and low and behold: no "x" on two of the tabs to close them out. Very strange.

This error is courtesy of the Mozilla Firefox automatic update feature. I believe it was encountered during last Tuesday's world record attempt of most downloads ever in a 24 hour period.
